What to expect during admission.

Admission to a short-term / long-term care facility is a process. The more you understand the process, the more comfortable you will be, especially if this is your first experience.

Assessment

Your loved one must qualify to come to a short-term / long-term care facility. We want to ensure we can meet their every need. An assessment will be done by the Social Director and a Nurse before they will be admitted.

Paperwork

Once accepted, you will need to fill out an admission agreement. When you come to your admissions appointment you will need to bring:

  • Medicare Card

and if applicable:

  • Secondary Insurance Card
  • Power of Attorney
  • Healthcare Proxy
  • State Issue ID
  • Rx Drug Cards
  • Living Will

Upon Arrival

Once our new Resident arrives, they will be shown to their room.

A physical assessment must be performed by the Treatment Nurse with the assistance of a Certified Nurses Assistant (C.N.A.). This is to verify the current condition of the Resident and is very important in establishing an initial Care Plan.

Moving In

Please bring things that are “home” to your loved one. We want them to be happy in their new home, surrounded by things that bring them joy. Photos are particularly important, especially for those with dementia, seeing themselves when they were younger can spark memories and conversation.

Be sure to label all clothes, shoes, etc.!!!

To comply with fire code, please maintain 12 inches ceiling clearance in the room and closet space. Please do not use the light fixture above the bed as a shelf.

Please do not bring

  • curling irons
  • heating pads
  • electric blankets
  • coffee pots
  • any item that produces heat or has hot surfaces

No knives or other weapons are permitted in Resident rooms.

Please do not bring in prescription or over-the-counter medications. Medications are strictly regulated by our Nursing Staff for the protection of our Residents.

You are welcome to bring in recliners, t.v.s, small refrigerators, dressers, etc. Please inventory all personal belongings with the Social Director.

Settling In

It will take a few days to get settled in, but within a week or so the routines become familiar, as will the faces and your loved one will be more comfortable. But, this does take some time, so please be patient with them, and with us as we get to know them better.
