    What is the Nurse / Staff / Resident ratio?

    Three Rivers Healthcare and Rehabilitation is licensed to accommodate 110 individual Residents, however, due to our full-private rooms, our building actually houses 106 Residents at full capacity. Based on a full capacity figure each Nurse will have 30-39 Residents in his or her charge, with an RN on staff daily to supervise. Two to three C.N.A.s are assigned to each hall (depending on the shift being worked). We also host a full service Dietary, Laundry and Maintenance Staff, as well as Activity and Administrative, and other personnel.

    What pharmacy does Three Rivers Healthcare and Rehabilitation use?

    AllCare Pharmacy.

    Please note: Medication may be given ONLY by an LPN or RN. Medication is given at scheduled times based on Physician orders. Under no circumstance may the Resident be allowed to have prescription or over-the-counter medication in their room.

    How will I be kept informed?

    The Primary Contact will be immediately notified in the event of an emergency, or significant change of condition.

    Of course, you are always welcome to visit with the Nursing Staff regarding your loved one's status. We also have scheduled meetings once a quarter, giving you the opportunity to sit down with several of the Department Heads to discuss your loved ones care. These meetings are called "Care Plans" and are designed to help staff and family with nursing care needs and the activities of daily living. Notices for the meetings are sent to the Primary Contact and attendance is strongly encouraged.

    When are bills due?

    Statements are sent by the first of each month. Please remit payment by the fifth.

    What are the visiting hours?

    Traditional visiting hours are daily from 7 a.m. to 9 p.m. We also allow sitters at the families expense but those sitters are not allowed to sit in the room while the roommate is receiving care or after 9 p.m.

    Can I take my loved one out of the facility?

    Yes, please check the Resident out at the Nurses Station, and sign for any medications you may need to dispense while you are gone (by signing out medication you assume full responsibility for it) and check the Resident back in upon return. Overnight stays longer than three days and nights require special consideration, so please contact the Social Director in these instances.

    Who is responsible for transportation should my loved one have a routine doctors appointment?

    Personal family physician visits are allowed and transportation will be provided by the facility at no charge if a van is not currently committed to another Resident.

    If you, the family member, make an appointment that is in conflict with a previously scheduled appointment for another resident the transportation will be your responsibility.

    All specialist appointment transportation will also be provided by the facility at no charge, when available, unless ambulance transport is required.

    Ambulance transport is subject to charge by the ambulance provider.

    Family members are welcome to take Residents out of the facility anytime and for any reason. Please remember to check out/in at the Nurse's Station.

    What about diet and nutrition?

    We make every effort to accomodate individual dietary needs and reqeusts.

    Our Dietitian will do an individual assessment to determine your loved one's likes and dislikes, and will also work with the doctor to ensure their needs are met.

    Three snacks are provided throughout the day and hydration is always a top priority.

    Please note: If the Resident is placed on a special diet by their Physician, the Dietitian or D.O.N will explain the reason for the diet and what foods are/ are not allowed. Physicians orders will be followed. Please check with the LPN or RN before taking food from outside the facility to a Resident. Some Residents may be refused certian foods due to their diet. Always check with a Nurse before giving another Resident any type of food or drink!

    May I dine with my loved one?

    Yes! If family trays are needed, the charge is $2.00 per tray. Our private dining room is available and may be reserved for family get-togethers. Please make your reservations for use of the private dining room with the Social Services Director. Tray request should be made with the Dietary staff.

    My loved one has fragile skin, do you have someone who specializes in skin care?

    Yes, Anna Rowland; LPN is our Treatment Nurse. She received her license in 2002. She works five days a week and is responsible for the care, prevention and treatment of skin tears, wounds and other skin conditions. She is also responsible for diabetic nail care.

    What about baths?

    As we get older, we become more fragile, susceptible to falls and our skin dries out more readily. With this in mind we have a dedicated bath team, whose sole responsiblity is to help meet the personal hygene needs of our Residents. Baths and showers are given three times per week, or based on need. Having the same team give your loved one a bath or shower builds trust and helps reassure the Resident that their modesty is respected.

    Do you have a fall prevention program?

    Yes, our "Falling Star" program discretely, respectfully and effectively identifies those at greatest risk for falls. We take great pride in our limited use of restraints, but, we also understand their necessity under certain circumstances.

    What if there is a weight loss?

    Three Rivers Healthcare and Rehabilitation is committed to the health of each Resident in our care. Weights are monitored monthly. If weight loss becomes an issue, the Resident is immediately placed on a restorative program and weight is monitored weekly until they are returned to their healthy weight. The primary contact will always be notified of any signifigant change.

    What about my loved one's Spiritual needs?

    We offer a wide variety of religious services and programs. Traditional Church services are provided a minimum of four times per week with additional programs throughout the month. Every effort will be made to provide specific support for any spritual need.

    Who is responsible for laundry?

    We have a full Laundry Staff on duty every day. Families who elect to do their own laundry will need to notify the Social Director and provide a hamper to be placed in the room.

    In either case- Please mark all clothes and other personal items (shoes, etc) with the Residents first initial and last name!

    Please make sure that any items you bring in after initial admission be documented in the Social Services Office on the inventory sheet in order to keep an accurate account of the residents belongings. We cannot be responsible for items not listed on the inventory sheet.

    Are pets allowed to visit?

    Yes!!! We love to see "grand- dogs & kitties"! Please be sure to keep them on a leash and have either tags or papers to verify current vaccinations. No animals are allowed in the dining areas.

    Concern / Complaint forms are available at the Nurse's Station or you may submit your concern online.
